Coming on February 20 – NextGen Office Clinical Content Update to the Patient Health Questionnaire-9 (PHQ-9)
The Patient Health Questionnaire-9 (PHQ-9) will be archived from Social History on February 20. The questionnaire is now available in Chief Complaint. You must manually activate this form.

To activate the PHQ-9 form in Chief Complaint
- Click Admin.
- Under EHR Setup, click Custom Forms.
- Under Form Manager, from the Filter menu, select Inactive.
- Click Questionnaire.
- Select the Active option for the PHQ-9 form.
To save a copy of NextGen Office-supplied custom forms
- Click Admin.
- Under EHR Setup, click Custom Forms.
- Under Form Manager, click Copy As New for the desired custom forms.
- Type the New Form Name.
- Click Save.
Note: It is your responsibility to maintain the accuracy of any form that you copy as new since after you save a copy, these forms are no longer maintained by NextGen Office.
